چکیده

Some 2500 children with congenital dislocation of the hip were treated at the Orthopaedic Hospital of the Invalid Foundation over the period 1954 to 1974 and have been regularly followed up to the end of growth. One of us (Laurent 1953) had already observed that the vulnerability of the femoral head had often led to changes resembling Perthes’ disease. In 1957 preliminary traction and adductor tenotomy was adopted as an essential part of the treatment, and from that time the incidence and severity of necrosis of the femoral head decreased markedly. Recently, neonatal diagnosis and treatment led to a remarkable change, the number of cases of established dislocation falling to between two and four a year compared with the previous average of 120. In cases developing Perthes’ disease, varus osteotomy was performed whenever a tendency to subluxation was observed-a practice similar to that of other workers (Lloyd-Roberts, Catterall and Salamon 1976). Epiphysiodesis of the greater trochanter to prevent severe coxa vara was also performed in a few cases (Laurent 1959). Over the twenty years, a severe form of late-onset Perthes’ disease developed in three children who had been successfully treated for congenital dislocation of the same hip. As this occurrence had not to our knowledge been previously described, we found it appropriate to present these cases.

گزارش یک مورد در رفتگی دو طرفه و غیر قرینه مفاصل هیپ

Introduction: Bilateral hip dislocation (especially asymmetric type) occurs rarely. By review of literature (published since 1951 till 2004), we found 15 cases of bilateral asymmetric hip dislocation (age between 3 and 36 years) that contained 12 cases in males.  All of the patients were treated using closed reduction and then traction. There was no considerable difference between the results o...

Complicated Congenital Dislocation of the Knee: A Case Report

Congenital dislocation of the knee (CDK) is a rare disorder. We report the case of a 7-year-old girl with bilateral knee stiffness, marked anterior bowing of both legs, and inability to walk without aid. Radiologic investigation revealed bilateral knee joint dislocation accompanied by severe anterior bowing of both tibia proximally and posterior bowing of both femur distally, demonstrating ...

Femoral Varus Osteotomy for Hip Instability after Traumatic Fracture Dislocations of the Hip Associated with Femoral Head Fractures: A Report of Two Cases

Fracture of the femoral head and the acetabulum with traumatic dislocation of the hip is a severe injury representing various types and unfavorable outcome. We showed a 45-year-old man with Pipkin type-IV fracture and coxa valga. An immediate closed reduction was achieved followed by open reduction and internal fixation via a posterior approach 6 days later. However, dislocation occurred three ...

Simultaneous of Mid Third Clavicle Fracture and Type 3 Acromioclavicular Joint Dislocation; A Case Report

Simultaneous mid third clavicle fracture and acromioclavicular joint dislocation is a rare combination injury, as a result of high-energy trauma. We report a patient with a middle third clavicle fracture and ipsilateral grade three-acromioclavicular joint dislocation, which is a rare combination. The patient wanted to get back to work as soon as possible, so the fracture was fixed with recons...

دررفتگی دورسال همزمان

Dorsal traumatic dislocation of metatarsophalangeal joint of great toe is a rare injury. Ability to reduce the dislocation by nonoperative measures depends largely on the type of dislocation and involvement of the sesamoid complex. There are three basic types of dislocations. Type I cases are usually irreducible on closed reduction, the metatarsal head being incarcerated by the conjoined tendon...
